The Arts and Community Exploration program is a people-centered interactive day program for individuals with intellectual and developmental disabilities. It focuses on the whole person, community involvement, and integration, with stimulating activities tailored to each participants needs and unique personality.

Family Support Services Program

The Family Support Services Program provides support for families who have children with developmental disabilities or delays with costs that are beyond those normally experienced by other families.

Eligibility Requirements:

The program is for families who have eligible children living at home or who are interested in having their child return from an out-of-home placement.

Program Options For Pasa Waivers & Private Pay

One-on-One Customized Activities

Schedule an occasional activity or one that is regularly repeated. Supplement a day program, achieve a particular participation or learning goal or give the primary care givers a break. Pricing depends on activity and can include all transportation in private vehicles.

Day Programs

Participate in a non-facility based day program one, two or three times a week. Your RecPartner will choose activities from within the community that create consistency and security but also provide variety and growth. Day Programs are available in groups or 1-3 people of similar ability and interests.

Special Events

Choose from a variety of additional activities including sporting events, concerts, movies, cultural events and a long list of physical activities such as bowling, swimming, biking, hiking, boating, tubing and skiing. Click this link for Current Schedule.

Travel Opportunities

Participate in overnight group travel opportunities to popular places in Colorado and neighboring states with occasional longer trips to other popular places. Colorado Indigent Care Program

The Colorado Indigent Care Program provides discounted health care services to low income people and families. CICP is not a health insurance program. Services vary by providers.

Who Qualifies?

  • You must be 18 and older
  • You must be at or below 250% of the Federal Poverty Level
  • You must be lawfully present in the United States and a legal resident of Colorado
  • You cannot be eligible for Health First Colorado or Child Health Plan Plus

Benefits and Services

  • Discounted health care services provided by participating Colorado hospitals and clinics
  • No premium costs
  • You are allowed to have primary health insurance or have Medicare
  • CICP ratings are good for a full year, see program information page for exceptions

Elderly Blind And Disabled Waiver

Waiver For Persons Who Are Elderly, Blind, or Disabled is a program to provide an alternative to nursing facility care for elderly, blind, or physically disabled persons, as well as individuals living with HIV/AIDS.

Eligibility Requirements:

In order to be enrolled in EBD you must:

  • either be 65 years of age and older with a functional impairment,
  • or be between the ages of 18 and 64 and be blind or physically disabled
  • be currently residing in, or be at risk of being institutionalized in a nursing facility or hospital
  • have an income less than 300% the Supplemental Security Income allowance per month
  • have countable resources less than $2000 for a single person or $3000 for a couple.

Breast And Cervical Cancer Program

The Breast and Cervical Cancer Program is for uninsured or underinsured women who have been diagnosed with breast or cervical cancer. BCCP also covers breast and cervical conditions that may lead to cancer if not treated.

Who Qualifies?

In order to apply for this program you must:

  • be diagnosed with breast or cervical cancer or conditions that may lead to cancer if left untreated
  • be between the ages of 40 and 64
  • have an income less than 250% of the Federal Poverty Level
  • not have health insurance that covers breast or cervical cancer treatments
  • not be currently enrolled in Health First Colorado and are not eligible for Medicare
  • be a U.S. citizen or a qualified non-citizen

  • breast reconstruction after breast cancer surgery

Health First Colorado Buy

Possibilities program helps special needs adults thrive

The Health First Colorado Buy-In Program for Children with Disabilities allows families who make too much to qualify for Health First Colorado and Child Health Plan Plus to buy into Health First Colorado coverage for their child with a disability by paying a monthly premium based on the family’s income.

Who Qualifies?

  • Children age 19 and under, with a qualifying disability per the Social Security Administration
  • A determination of disability by the SSA is accepted as proof of disability
  • Children not certified through the SSA can still be determined disabled by the State by completing the Health First Colorado Disability application
  • Family income must be below 300% of Federal Poverty Level , after income disregards are calculated

Early And Periodic Screening Diagnostic And Treatment

The Early and Periodic Screening, Diagnostic and Treatment benefit provides comprehensive and preventive health care services. EPSDT is key to ensuring that children and youth receive appropriate preventive, dental, mental health, developmental and specialty services.

All Health First Colorado coverable, medically necessary services must be provided even if the service is not available under the state plan to other people who qualify for Health First Colorado. Benefits not listed are not considered to be a state plan benefit and are therefore outside of EPSDT coverage and exceptions. No arbitrary limitations on services are allowed, e.g., one pair of eyeglasses or 10 physical therapy visits per year.

Who Qualifies?

Children and Youth ages 20 and younger who are enrolled in Health First Colorado.

The Children’s Home and Community-Based Services Waiverprovides a home or community based alternative to children with significant medical needs who are at risk for acute hospital or skilled nursing facility placement.

Who Qualifies?

  • Medically fragile children ages 17 and younger who are in the home and at risk of nursing facility or hospital placement
  • Child must be institutionalized or at risk of being institutionalized in a nursing facility or hospital level of care
  • Children’s income must be less than 300% the Supplemental Security Income allowance per month and countable resources less than $2,000

Supported Living Services Waiver

The Supported Living Services Waiver is a program to provide supported living in the home or community to persons with developmental disabilities.

Who Qualifies?

In order to be enrolled in SLS you must:

  • Be 18 years of age and older
  • Be able to live independently with limited supports or, if you need extensive supports, are already receiving that high level of support from other sources, such as your family
  • Be currently residing in, or be at risk of being institutionalized in an Intermediate care facility
  • Have income less than 300% the Supplemental Security Income allowance per month
  • Have countable resources less than $2,000 for a single person or $3,000 for a couple.

Colorado Dental Health Care Program For Low

The Colorado Dental Health Care Program for Low-Income Seniors provides discounted dental health care services to individuals 60 years of age or older whose income is insufficient to meet the costs of such care.

Who Qualifies?

  • You must be 60 or older
  • You must be at or below 250% of the Federal Poverty Level
  • You must be lawfully present in Colorado
  • You cannot be eligible for dental services under Health First Colorado or the Old Age Pension Health and Medical Care Program
  • You cannot have private dental insurance

Benefits:

  • Discounted dental health care services provided by participating Qualified Grantees and Providers
  • Services include, but are not limited to: oral examinations, x-rays, partial and full dentures, repair of permanent teeth, removal of permanent teeth, root canals, crowns, and much more

Personal Care Alternative Program

This highly individualized program helps a person with a developmental disability live in his or her own home or a host home. Staff members are available for daily, weekly, or monthly support while providing training in daily living skills, money management, meal preparation, and community and safety skills. The goal of this program is to offer the client support so that he or she can have a quality life style in a typical neighborhood setting, taking advantage of what the community has to offer.

What We Do:

  • Teach daily living and domestic skills in a persons own home.
  • Provide follow-along support for money management and medical appointments.
  • Provide 24 hour a day phone support.
  • Advocate with day programs and other service providers.
  • Participate in recreation and leisure skill development.
  • Follow through on Individual Service and Support Plan goals.

Adult Day Program In Aurora

Esther Houseoffers adults with disabilities the opportunity to live, learn, work and play as a vital member of their own community.Our Adult day program in Aurora ensures that people with developmental disabilities take on valued roles in the community by providing services and training that selects individual choices and opportunities.

Every weekday, our Adult day program in Aurora accommodates up to 20 guests. The Program enhances the well-being of seniors through a full range of recreational, social and therapeutic programs.

In Adult Day Services in Aurora, participants find pathways to independence, build confidence, explore their community and develop independent living skills. Weekly plans are developed with individual input.

Program participants are able to discover new abilities in a highly encouraging and inclusive atmosphere. Small group ratios and a highly trained staff also provide an environment for growth!   • Require moderate to total assistance in at least three of the18 Personal Care Tasks

Children’s Extensive Support Waiver

Eligibility Requirements:

  • Children 18 years of age and younger with intensive behavioral or medical needs who are at risk of out-of-home placement and have been determined to have a developmental delay/disability through a Community Centered Board

Here Is How We Do More

  • Wide variety of daily activities, including games and specifically tailored cognitive skills activities, an exercise program , as well as other therapeutic recreation activities. Additionally, there are a number of large screen TVs available in various entertainment rooms for clients who prefer to watch movies / news / shows from time to time. A billiard room is also available on-site for our client’s use.
  • Included in our program are daily local trips to grocery shops, malls and, weather permitting, parks and weekly / bi-weekly excursions to museums, the botanical gardens, libraries, the Denver Zoo, hot springs, and other locations of interest .
  • We host birthday parties on a monthly basis to celebrate the birthdays of all our clients with live music, dancing, birthday cake and small gifts!
  • We care for those with Parkinson’s disease, Alzheimer’s, stroke, brain injury, developmental disability and other conditions.

Adult Day Program. Our adult day program is guided by family, medical professionals and caregivers, we learn about each clients special needs, even before acceptance in our program.

Programs Available

Our programs offer adults with developmental disabilities a comfortable group setting which is a positive alternative to a nurse assisted home care service. We provide social and health services, fun and engaging Adult Day Program Activities, gentle exercise, warm water therapy pool, meals and field trips. All activities are designed for People with Developmental Disabilities. We also provide transportation in the Denver area. Working together as a unified team, we design a program customized to their own physical, emotional, sociological, medical and nutritional needs. As well as social outings. Caregiver respite.
