Rock County Aging And Disability Resource Center Will Host Online Weight

Accessing Texas Department of Aging and Disability Services

The Rock County Aging and Disability Resource Center is partnering with North Carolina State University to offer two programs a part of the Eat Smart, Move More, series, with one being titled Weigh Less and the other named Prevent Diabetes. The ADRC has offered Weigh Less in the past and decided to add Prevent Diabetes this year, Aging Health Promotion Coordinator Lisa Messer said. Prevent Diabetes is structured to help those who have a pre-diabetes diagnosis. Both programs offer multiple online classes.

Messer said the goal of the programs is to help people with common New Years resolutions lose weight, increase activity and get blood sugar under control.

The truth of the matter is those resolutions often fall flat due to lack of support and waning motivation, Messer said.

Each program will be online. Weigh Less had 20 seats available as of this past Tuesday, Dec. 20. Five seats were available for Prevent Diabetes as of Tuesday, Dec. 20. They are being offered for suggested donations of either $25 or $45.

Both programs are online and can be done on an on-demand basis and will not have in-person sessions. For questions or to register for either program, contact Messer at 608-757-5309 or . A code will be given at registration. Participants must be at least 60 years old who live in Rock County and have access to a working computer or tablet with internet access. Participants are encouraged to register by Dec. 30.

The Health Promotion Program has taken a step towards a proactive model of care in the use of an evidenced-based chronic disease self-management program known as “Living Well.” The “Living Well” program is an opportunity for County seniors to take a proactive stance in their care through a 6-week workshop that creates a person-centered partnership in care management.

Caregivers May Apply For Temporary Voucher Program For Respite

November is National Family Caregivers Month. DARS recognizes the tremendous effort and care that one in five Virginians provide loved ones, often around the clock. Virginia families who care for a loved one with disabilities or chronic conditions can now apply for up to $400 reimbursement to get help taking a break from those duties, under a limited respite care voucher program from DARS.

Eligibility

The majority of services provided through the Area Agency on Aging and Disability are for persons over age 60 who are in greatest economic and social need. With the Options for Community Living Program, the AAAD may serve persons with disabilities 18 years of age and older based on a functional needs assessment.

    As a regional office, we administer funds to provide group and home delivered meals, homemaker services, personal care services, legal services, ombudsman services, limited transportation services, information and assistance services, case management, Medicare insurance counseling services, health promotion services, senior centers, county offices on aging, and public guardianship services. These services are provided directly and through a number of service provider contracts in the region.

    Aging And Disability Resource Centers

    The Department of Elder Affairs administers programs and services for elders across the state of Florida through 11 Area Agencies on Aging, which operate as Aging and Disability Resource Centers . These ADRCs function as a single, coordinated system for information and access to services for all Floridians seeking long-term care resources.     Meals on Wheels will continue to deliver hot and/or frozen meals, five days a week according to each clients meal-delivery schedule.

    Outdoor Events feature live music and exercise classes. Participants must register ahead of time for classes and order boxed lunches in advance. The Office on Aging & Disability Services is open to walk-ins and curbside assistance. A clients need for curbside service or walk-in appointment will be determined over the phone by staff.     Guardianship serves society’s most vulnerable individuals who need assistance in making decisions about their health, lives, and finances and who may be at risk of abuse, neglect and exploitation.

    Our agency provides public guardianship services for individuals who are: 18 years of age and older incapacitated indigent and have no willing and suitable person to serve as their guardian.

    Ageing, Disability and Home Care NSW was a division of the Department of Family and Community Services in the Government of New South Wales that is responsible for the provision of services to older people, people with a disability, and their families and carers in the state of New South Wales, Australia.

    The ADHC division supports approximately 300,000 vulnerable people from its annual budget of A$2.5 billion. It employs more than 13,000 staff. ADHC delivers services and manages 900 non-government andlocal government service providers that are funded to deliver services such as early intervention, therapy, respite, supported accommodation, case management and post-school programs.

    The division is led by a Deputy Secretary, presently Jim Longley, who reports to the Minister for Ageing and Minister for Disability Services, presently the Hon. John Ajaka MLC.

    Area Agencies On Aging

    The five Area Agencies on Aging in Maine serve as one-stop-shops to answer questions from older adults, individuals with disabilities and their care partners about a wide range of in-home, community-based, and institutional services.

    Area Agencies on Aging are experts at answering questions about in-home care services and other types of long-term support. Maines five AAAs are also Aging and Disability Resource Centers that:

    • provide information and assistance to individuals needing either public or private long-term care resources,
    • serve professionals seeking assistance on behalf of their clients long-term care needs,
    • serve individuals planning for their future long-term care needs, and
    • serve as the entry point to publicly administered long-term supports including those funded under Medicaid, the Older Americans Act and state funded programs.

    Oregon Aging and Disability Resources Connection Website Overview

    In 2004 under a special report to the NSW Parliament, the New South Wales Ombudsman raised concerns about the way services to children and young people with disability were being provided in NSW. The Ombudsman found that for families seeking support to care for children with disabilities, there was lack of clarity about how to access support and that the service system was fragmented and was characterised by poor coordination of support. A subsequent progress report in 2006 highlighted that while progress had been made to improve systems to support the provision of disability services, there had been no evaluation of the longer term impacts of the reforms on families requiring those services. In response to these reports, the Iemma government released Stronger Together, a 10-year plan to improve and expand services to people with disability and their families. The first five years were funded through an investment of A$1.3 billion and set out plans to increase capacity of the disability service system by 40 per cent. A subsequent four-year plan, released in February 2007 entitled Better Together, focused on improving universal and adapted services: such as childcare school before and after school care and vacation care.

    Families told us that in their experience many health practitioners, social workers, and even some ADHC staff, lack knowledge about the specialist disability service system.
