If you are requesting the wheelchair accessibility symbol on the license plate, you must also submit an original, signed Physicianâs Statement for Disabled License Plates/Placard from an advanced practice registered nurse, licensed physician, chiropractor, podiatrist, physical therapist, or optometrist, stating that the disability is permanent. The physicianâs statement is valid for 90 days after completion by the physician.
